We will reserve a time that is convenient for you. Once your reservation has been made, you will receive Medical History Forms to complete online prior to your visit. Completing the forms prior is required as it helps us plan and prepare a treatment plan as well as allow adequate time for you. You will also receive a link to join the visit. Video visits are easy to join and can be done from your home, office, or on the road. Just be sure you allow enough time and try to minimize distractions. We will review your Medical History in detail and answer any questions you may have, preparing your questions ahead of time will make the most out of our visit.
